Additions to PDAC Product Sample Requirements


Effective March 1, 2021, a product sample must be included with any Code Verification Review Applications submitted to PDAC for the following orthotic devices.

Code LCD/Policy Group Description
L1005 Orthotics - SO TENSION BASED SCOLIOSIS ORTHOSIS AND ACCESSORY PADS, INCLUDES FITTING AND ADJUSTMENT
L1690 Orthotics - Lower Limb COMBINATION, BILATERAL, LUMBO-SACRAL, HIP, FEMUR ORTHOSIS PROVIDING ADDUCTION AND INTERNAL ROTATION CONTROL, PREFABRICATED, INCLUDES FITTING AND ADJUSTMENT
